I’ve found the Adult Social Care so helpful in the role I do. It saves me hours of chasing up if people have referrals, who their social worker is, if they have one allocated and where are they based etc.

I found it very helpful recently to be able to view the Drs consultation/discussion notes with the patient and their family. This enabled me to know the patient was being referred for fast track/end of life care planning. Without this information, I would have still been ringing the ward requesting an update.
